Be as specific as you can with your description, and include any expiration dates, restrictions, or pertinent information. If you are filling out this form for multiple items, please include the estimated value for each item in the description. Estimated values for tax deduction are the responsibility of the donor.
If you are donating alcohol of any kind: The Ohio Department of Liquor Control requires that any spiritous liquor, wine, or beer that is donated to Ursuline Academy for the purpose of this fundraising event must be accompanied by a receipt of purchase.
